All dancers MUST have a Kent School District Athletic Department Participation History and Physical Examination Form on file to participate.  Incoming 7th graders must have the form filled out after June 1st of their 7th grade year.  The form is valid for 24 months.  8th graders can have their form filled out anytime after June 1st of their 7th grade year.  
The form used must have the correct school year in the top, right-hand side.  It is usually updated on June 1st.
